    I wanted to send this note earlier but with the current situation and lockdown I’ve not been able to do so. My apologies for that.

    After 3 to 4 months of critical care, my father in law passed away earlier this month. We as a family are extremely thankful to the nursing staff who were with him throughout and did everything possible to make him feel comfortable and cared for.

    We did hit a rough patch at the start but your intervention helped a lot and we’ve not had to worry about quality thereafter.

    While there were quite a few staff members who were involved (Laxman, Monto, Ebby) and very good at their work, in particular we have a deep sense of gratitude for Mr. Jabin who has consistently been with him from day one. Mr. Jabin was assigned to take care of my father in law while he was still in hospital and he took all efforts to match the quality of care just as they were doing in the hospital. Till the last moment, he was there with our family and has gone above and beyond his duties. It was extremely touching to see how he developed a bond with his patient and would care for him as one of his own. The more I say is less when it comes to Jabin and the way he has taken care of him is exemplary.

    We are very thankful to the entire Portia team. Your services are a blessing for families that are need of critical care in a home setup.

  • I would like to appreciate the support provided by the Portea Luckow team during this crisis time.
    I have been taking services from your organization for almost over a year now, have faced few minor issues earlier but the kind of support the team provided in this testing time when the whole medical support is closed is exceptional and laudable.
    My father was admitted in hospital on 20th March and was in ICU and was forcibly discharged on 25th March while being monitored in ICU. I was not getting any support from any of the hospital then I called Rakshit for help. He immediately arranged for a 24 hrs nurse assistant at home. He gave me confidence to overcome the change of managing my father at home. He has been in constant touch with me providing help, guidance and facilitating medical supplies at home. Looking at the criticality of the patient he made a home visit and took cognizance of things , guided the NA asked him to take proper care and even help my mother in home care also.

What Is a Tracheotomy?

Tracheotomy also referred to as Tracheostomy is basically a surgical procedure that is used to create a permanent opening called tracheostomy into the trachea (windpipe) from an incision on the anterior aspect of the neck for the installation of a breathing tube. Tracheotomy helps a patient to breathe through the tracheostomy tube fitted in the trachea, thereby bypassing the nose or mouth. Tracheotomy is helpful in cases where the upper respiratory tract is obstructed.

Difference between Tracheotomy and Tracheostomy

The terms Tracheotomy and Tracheostomy are often used interchangeably but, there is a difference in their meaning. While tracheotomy refers to the surgical procedure and incision into the trachea, the term tracheostomy refers to the physical opening that allows breathing and removal of secretions from the lungs.

Tracheotomy Treatment

Tracheostomy is an important surgical procedure that is used in the treatment of several health complications like, in an emergency situation when the upper respiratory tract is blocked, a tracheotomy is performed to bypass the obstruction in the upper airway and provide oxygen to the lungs or to remove secretions from the lungs.

Some of the health conditions that require tracheostomy are as follows;

  • When the upper respiratory tract is blocked by any foreign object
  • In case of injury to the larynx, neck or mouth
  • In cases of severe infection in the upper respiratory tract like in the epiglottis or croup
  • Presence of tumor in the upper respiratory tract that is causing blockages in breathing normally
  • To treat Obstructive Sleep Apnea
  • In the treatment of Chronic Pulmonary Disease
  • In the dysfunction of the diaphragm
  • Anaphylaxis
  • Coma

Tracheostomy Procedure

Tracheotomy is performed in a safe and sterile environment of a hospital with the patient under general anesthesia.
The patient’s neck is extended over the shoulder roll and the endotracheal tube is so positioned that its cuff is at level with the middle of the vocal cord.
After that an incision of about 3-4 cm is made into the inferior border of the cricoid cartilage. The thyroid isthmus is then pushed down with unobtrusive dissection of the paratracheal tissue. The next step is the stabilisation of the larynx, after that a bronchoscopy is performed. A dilater is then stacked and used to enlarge the tissue between the skin and tracheal lumen, setting up the zone for the arrangement of the tracheostomy tube.
Post-operative care is then advised to patients to aid the recovery process following the surgical procedure and reduce the risk of infections and complications.

Tracheostomy Complication

Although complications are a rarity but, still there are a few cases of complications reported after tracheotomy. Some of the common tracheotomy complications are as follows;

  • Bleeding of the trachea or the tracheostomy
  • Pneumothorax or trapping of air resulting in collapsed lungs.
  • Pneumomediastinum or subcutaneous emphysema
  • Difficulties in speaking and swallowing due to damage to the esophagus or larynx
  • Blockage of tracheostomy tube by blood clots or mucus
  • Accidental decannulation or removal of the tracheostomy tube
  • Infection of the trachea
  • Tracheomalacia
  • Tracheo-esophageal fistula
  • Granulation
  • Difficulty breathing due to the narrowed or collapsed esophagus

Tracheostomy Patient Care

Tracheostomy care is crucial as improper or lack of care can lead to infections and difficulty in speaking, swallowing and breathing among other complications. Tracheotomy patients need proper tracheostomy nursing care due to various reasons. Tracheostomy care mainly involves suctioning to prevent occlusions and replacing supplies and removing secretions. Suctioning is only performed when it is clinically necessary due to the risks involved like, hypoxia or collapsing lungs and infection.
